New obesity management guidelines, including those from the American\u00a0<\/span>Diabetes Association, advocate the use of shared decision-making (SDM)\u00a0<\/span>for obesity treatment. SDM is an evidence-based approach for promoting\u00a0<\/span>person-centered care and is widely recommended across medical\u00a0<\/span>specialties. However, a variety of issues challenge the use of SDM in\u00a0<\/span>daily practice. In this narrative review we discuss factors that may\u00a0<\/span>impede the use of SDM in adults with obesity and type 2 diabetes, as\u00a0<\/span>well as past efforts to address these barriers. Patient-level factors\u00a0<\/span>such as internalized weight bias and lack of knowledge about obesity and\u00a0<\/span>its treatment, provider-level factors such as limited training in\u00a0<\/span>obesity management and lack of confidence in sensitively discussing\u00a0<\/span>weight, and system-level factors like poor treatment access and limited\u00a0<\/span>care coordination stymie effective SDM around obesity management. A<\/span>
perceived power imbalance between patients and providers and medical\u00a0<\/span>mistrust are additional barriers for some. In the past, researchers have\u00a0<\/span>attempted to overcome these barriers to advancing SDM through\u00a0<\/span>approaches including patient decision aids, provider training, and\u00a0<\/span>clinical decision support systems, with moderate success. This article\u00a0<\/span>concludes with recommended strategies for clinical adoption of SDM for\u00a0<\/span>patients with obesity and type 2 diabetes, a call for system-level\u00a0<\/span>changes to create an environment more conducive to effective SDM, and\u00a0<\/span>directions for future research.<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t
