Biohazardous Waste Collection Request Biohazardous Waste Collection Request This form is to be used for Temple University Main Campus waste pick-up locations for packaged biohazardous waste.Step 1. Verify that all containers meet the following conditions:Biological waste boxes must be lined with an EHRS supplied red bag and the bag must be tied in a knot before taping shut. Biological waste boxes must be taped shut on all open edges before requesting a pickup.Sharps containers must be properly closed and must fit inside a biological waste box.Biological waste boxes must not weigh more than 45 pounds, be wet or leaking, or be otherwise breached.Each biological waste box must be labeled with an EHRS supplied sticker that includes the following information: Generator Name, Contents, Building, Date, and Room Number. Any boxes not in compliance with these conditions will NOT be picked up by EHRS until they are in compliance.Step 2.
